The penile prosthesis implant is indicated for men who suffer from sexual impotence, which is the capacity of a man, obtain and/or keep an erection long enough for the penis to have complete sexual intercourse. Complete sexual intercourse is defined as lasting from vaginal penetration to ejaculation.
The mechanism of erection is fairly complex and depends on psychological, neurological and circulation factors occurring. Erection is indicated by psychogenic stimuli, for example, desire and sexual fantasies, and reflexogenics, overall, on touch in the genital area. These stimuli trigger the increase of flux in the penile region, which result in erection.
Many medications can reactivate the erection mechanism, but when it is not possible we still have a very efficient tool which is the penile prosthesis. It always causes a lot of apprehension when we indicate the use of a penile prosthesis. The patient in general does not want surgery to solve this "little problem" with prosthesis and the female partner feels constraint initially. However, after its placement, the index of satisfaction is very high reaching 95% for the couple in our statistics.
The penile prosthesis was developed in the 70s decade. It is made of silicon and possesses a small, flexible metallic filament inside. There are many types of penile prosthesis on the market, with various sizes, different angles, etc. Some penile prosthesis are activated by a kind of air or liquid "bag ", implanted along with the prosthesis, and pumped by the patient.
The penile prosthesis is placed through surgery using a urethral catheter and the anaesthesia is by epidural blocking or raqueanestesia. In special cases, general anaesthesia can be indicated.
